Whole Foods Market ® in Hadley, Massachusetts is issuing an allergy alert to recall six-packs of Banana Chocolate Chip Muffins due to an in-store labeling error.
The muffins, which were labeled as “Made Right Here 6pk Banana Choc Chip Muffins”, contained walnuts, which were not declared on the ingredient list. People who have an allergy or severe sensitivity to walnuts run the risk of serious or life-threatening allergic reaction if they consume these products. The muffins were sold in clear, plastic six-pack containers and had a “Sell by” date of March 28, 2017.
Signage is posted to notify customers of this recall.
One allergic reaction has been reported.
Consumers who have purchased this product may bring their receipt to the store for a full refund. Consumers with questions should contact the store at 1-413-586-9932.

WASHINGTON, March 24, 2017 – Lakes Farm Raised Catfish Inc., a Dundee, Miss. establishment, is recalling approximately 1,695 pounds of siluriformes fish (catfish) products that may be adulterated with residues of public health concern, specifically Malachite Green and Leucomalachite Green, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.
The siluriformes (catfish) products were produced on March 14 through 17, 2017. The following products are subject to recall: [View Labels (PDF Only)]
  • 15-lb. cardboard boxes of frozen raw “catfish” fillets identified as LAKE’S FARM RAISED CATFISH and having lot codes T-14, T-15, T-16, and T-17.
  • 15-lb. cardboard boxes of frozen raw “catfish” nuggets identified as LAKE’S FARM RAISED CATFISH and having lot codes T-14, T-15, T-16, and T-17.
  • 15-lb. cardboard boxes of frozen raw “catfish” steaks identified as LAKE’S FARM RAISED CATFISH and having lot codes T-14, T-15, T-16, and T-17.
  • 15-lb. cardboard boxes of frozen raw whole “catfish” identified as LAKE’S FARM RAISED CATFISH and having lot codes T-14, T-15, T-16, and T-17.
The products subject to recall bear establishment number “EST. 48150” inside the USDA mark of inspection. These items were shipped to institutional and retail locations in Mississippi and Tennessee.
The problem was discovered on March 23, 2017, after routine FSIS sampling results revealed violative levels of the chemicals Malachite Green and Leucomalachite Green in the products.
There have been no confirmed reports of adverse reactions due to consumption of these products. Anyone concerned about an injury or illness should contact a healthcare provider.
Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.

WASHINGTON, March 24, 2017 – Ajinomoto Windsor, Inc., a Lampasas, Texas establishment, is recalling approximately 35,168 pounds of frozen ready-to-eat beef taquito products that may be contaminated with extraneous materials, specifically rubber with plastic, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.
The frozen ready-to-eat beef taquito items were produced on December 30, 2016. The following products are subject to recall: [View Labels (PDF Only)]
  • 60-oz. plastic bags inside of a corrugated carton labeled as “JOSÉ OLÉ TAQUITOS BEEF CARNE DE RES IN CORN TORTILLAS Crispy and Crunchy,” with case codes 3366365A, 3366365B, 3366365C, 3366365D and a Best By date of December 30, 2017.
The products subject to recall bear establishment number “Est. M-5590” inside the USDA mark of inspection. These items were shipped to retail locations in California, Florida, Illinois, Missouri, New York, Ohio, Pennsylvania, Texas, and Wyoming.
The problem was discovered when Ajinomoto Windsor, Inc. received two consumer complaints of foreign material in its ready-to-eat beef products on March 14, 2017 and March 21, 2017. The foreign materials were pieces of rubber with white plastic that originated from the establishments processing equipment.
There have been no confirmed reports of adverse reactions due to consumption of these products. Anyone concerned about an injury or illness should contact a healthcare provider.

WASHINGTON, March 23, 2017 – OK Food, Inc., an Oklahoma City, Okla. establishment, is recalling approximately 933,272 pounds of breaded chicken products that may be contaminated with extraneous materials, specifically metal, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.
The ready-to-eat (RTE) breaded chicken items were produced on various dates from Dec. 19, 2016 through March 7, 2017. A list of the products subject to recall can be found here PDF | View Labels.
The products subject to recall bear establishment number “P-7092” inside the USDA mark of inspection. These items were shipped to retail locations and institutions nationwide.
The problem was discovered on March 21, 2017 after OK Foods Inc. received five consumer complaints stating that metal objects were found in the ready-to-eat chicken products and by FSIS inspection personnel during verification activities. After an internal investigation, the firm identified the affected product and determined that the objects in all the complaints came from metal conveyor belting.
There have been no confirmed reports of adverse reactions due to consumption of these products. Anyone concerned about an injury or illness should contact a healthcare provider.

March 23, 2017 - Pro Sports Club of Bellevue, WA is recalling 36,957 Yogurt Peanut Crunch bars because it may be contaminated with Escherichia coli O157:H7 bacteria (E. Coli O157:H7). E. coli O157:H7 causes a diarrheal illness often with bloody stools. Although most healthy adults can recover completely within a week, some people can develop a form of kidney failure called Hemolytic Uremic Syndrome (HUS). HUS is most likely to occur in young children and the elderly. The condition can lead to serious kidney damage and even death.
The Yogurt Peanut Crunch Bar was sold directly to clients at the PRO Sports facilities in Washington (Bellevue, Redmond and Seattle cities) between August 8, 2016 and March 10, 2017. Product was also sold to internet customers through www.2020lifestyles.com.
This recalled Yogurt Peanut Crunch Bars has a brand name 20/20 Life Styles, lot code B.B. 22JUL17 (means best by July 22, 2017), net weight 2 oz., UPC Code 78571 00052.
No illnesses related to the consumption of the Yogurt Peanut Crunch Bars have been reported to date.
The recall was initiated after our manufacturer notified us that the ingredient used in the Yogurt Peanut Crunch Bars was recalled by supplier Soy Nut Butter Company of Glenview, Illinois because it was found to contain E. Coli O157:H7.
This recall is being made with the knowledge of the U.S. Food and Drug Administration.

ChloroFields of Lawrence, KS is recalling 20 cases of Asian Microgreens, because they have potential to be contaminated with Salmonella, anorganism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea, nausea, vomiting, and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.
The products were distributed throughout Colorado, Kansas, and Missouri, and were made available through retail grocery stores.
The product comes in a 1.5 oz clamshell, and is identified as Asian Mix Microgreens with a sell by date of 3/26/2017. The sell by date is located on a white sticker on the bottom of the container. The UPC number of the product is 853763007096.
No illnesses have been reported to date.
The recall was a result of routine examinations conducted by the FDA that revealed finished product that contained the bacteria. Chlorofields along with the FDA is continuing investigation as to what caused the problem, and has ceased distributing the product.
