Sliced Strawberry Swirl Crème Cake with a production code of 16098" ink jet coded on plastic clam shell package, due to a potential presence of peanut allergen in a CSM Bakery Solutions supplied ingredient. People who have allergy or sensitivity to peanuts run the risk of serious or life threatening allergic reaction if they consume these products.
The recalled product was distributed in multiple Retail Stores in Alabama, Florida, Georgia, Illinois, Indiana, Kentucky, Louisiana, Maryland, North Carolina, Ohio, Pennsylvania, South Carolina, Tennessee, Texas, Virginia, and West Virginia.

The recall was initiated after Old Home Kitchens was informed by CSM Bakery Solutions that an ingredient supplied to Old Home Kitchens had a potential presence of a peanut allergen. These products do not reveal the presence of peanuts. No illnesses have been reported to date.

Old Home Kitchens has alerted the Distributers of these Retailers, which has removed this product from its stores. The recall is being made with the knowledge of the U. S. Food and Drug Administration.


Consumers who have purchased "Old Home Kitchens 14oz Sliced Crème Cake" with the codes noted are urged to return it to their Retail Stores for product replacement or refund. No other product is included in this recall.

Glenn Wayne Wholesale Bakery of Bohemia, NY, is recalling select 7-ELEVEN FRESH TO GO brand cookies because they may contain undeclared peanuts. People who have an allergy to peanuts run the risk of serious or life-threatening allergic reaction if they consume these products.
The select 7-ELEVEN FRESH TO GO Cookies were sold at 7-ELEVEN stores located throughout New York State.

The recall was initiated after one of our suppliers reported that three different cookie mixes may contain undeclared peanuts. These potentially contaminated cookie mixes were used in production and distributed in packaging that did not reveal the presence of peanuts. The company and the ingredient supplier continue their investigation to determine the cause of the problem.

WASHINGTON, April 26, 2016 – Pilgrim’s Pride Corp., a Waco, Texas establishment, is recalling approximately 4,568,080 pounds of fully cooked chicken products that may be contaminated with extraneous materials, including plastic, wood, rubber, and metal, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.

On April 7, 2016, Pilgrim’s Pride Corp recalled approximately 40,780 pounds of fully cooked chicken nuggets produced on Oct. 5, 2015. The following product is subject to recall:

20-lb. cardboard boxes containing two, 10-lb. clear plastic bags of fully cooked chicken nuggets labeled as “GOLD KIST FARMS Fully Cooked Whole Grain Popcorn Style Chicken Patty Fritters” with package codes 5278105021, 5278105022, 5278105023, 5278105000, and 5278105001.
The products subject to recall bears establishment number “EST. 20728” inside the USDA mark of inspection. These items were shipped for institutional use nationwide. According to Pilgrim’s Pride Corp. records, schools have purchased products through the company’s commercial channels.

The problem was first discovered after the firm received several consumer complaints regarding plastic contamination of the chicken nuggets. The firm notified FSIS personnel of the issue on April 6, 2016. FSIS personnel identified more affected product types and dates of production after investigating additional consumer complaints of foreign material contamination received by the recalling firm.

There have been no confirmed reports of adverse reactions due to consumption of these products. Anyone concerned about an injury or illness should contact a healthcare provider.

Consumers who have purchased the recalled products are urged not to consume them. These products should be thrown away or returned to the place of purchase.

FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers of the recall and that steps are taken to make certain that the products are no longer available to consumers. Krasnyi Oktyabr Inc. in Brooklyn, NY, is recalling Vobla (Roach) Dry Eviscerated Salted Fish and Bream Dry Eviscerated Salted Fish because they have the potential to be contaminated with Clostridium botulinum, a bacterium which can cause life threatening illness or death. Consumers are warned not to use the products even if they do not look or smell spoiled.

Botulism, a potential fatal form of food poisoning, can cause the following symptoms: general weakness, dizziness, blurred or double-vision and trouble with speaking or swallowing. Difficulty in breathing weakness of other muscles abdominal distention and constipation may also be common symptoms. People experiencing these problems should seek immediate medical attention.

The products were imported from Russia and sold to retail stores in New York and California from February through April 2016.

The products are sold in un-coded clear, vacuum pack plastic bags, each weighing 5.29 ounces:

VOBLA (ROACH) DRY EVISCERATED SALTED FISH, UPC 4606495-001308;BREAM - DRY EVISCERATED SALTED FISH, UPC 4606495001346

The potential for contamination was noted after routine sampling and inspection by the US Food and Drug Administration and subsequent analysis of the Bream fish by the FDA confirmed that the Bream fish was not properly eviscerated prior to processing.

No illnesses have been reported to date in connection with this problem.

CVS Pharmacy Announces Voluntary Recall of Gold Emblem Abound Organic Spiced Herbal Tea Due to Possible Health Risk

CVS Pharmacy® announced today that it has voluntarily recalled select cases of Gold Emblem Abound™ Organic Spiced Herbal Tea following notification from the manufacturer that the product is potentially contaminated with Salmonella. An ingredient produced by the manufacturer’s raw material supplier tested positive for Salmonella in another company’s product. CVS Pharmacy’s Gold Emblem product also contains this ingredient.
The product is labeled “Gold Emblem Abound Organic Spiced Herbal Tea 1.41 oz” and was packed in 1.4 oz cartons. The recalled product has a single best by date of 18 Mar 2018 with a UPC code 0 50428 541043. No other best by dates are affected. The product was available at CVS Pharmacy stores nationwide.

Salmonella is an organism that can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.

Approximately 200 units of the recalled product were sold prior to this recall. The Company is not aware of any reported cases of illness to date related to this product and it has issued this voluntary recall as a matter of precaution.

CVS Pharmacy has removed the affected products from its stores and the manufacturer has notified the U.S. Food and Drug Administration. A “do not sell” block has also been placed in the Company’s store register system to prevent further sale of these recalled items.

CRF Frozen Foods of Pasco, Washington is voluntarily recalling fifteen frozen vegetable items that have the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, listeria infection can cause miscarriages and stillbirths among pregnant women.
No illnesses have been reported to date, but the company is recalling the products as a precaution. The Listeria was discovered through routine testing by state health officials in Ohio. Listeria monocytogenes was found to be present in one lot of Individually Quick Frozen (IQF) organic petite green peas and one lot of IQF organic white sweet cut corn.

WASHINGTON, April 21, 2016 – Meating Place, a Buffalo, N.Y., establishment, is recalling approximately 325 pounds of ground beef product that may be contaminated with E. coli O157:H7, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.

The ground beef patties were produced on April 15, 2016. The following product is subject to recall: [View Label (PDF Only)]

5-lb. boxes of “LORINGO BRAND BEEF PATTIES KEEP FROZEN THE MEATING PLACE INC.” with a package code of 120106.
The product subject to recall bears establishment number “Est. 8631” inside the USDA mark of inspection. The item was shipped to retail and intuitional locations in the Buffalo, NY area.

The problem was discovered by FSIS during routine in-plant review of testing results. There have been no confirmed reports of adverse reactions due to consumption of this product.

E. coli O157:H7 is a potentially deadly bacterium that can cause dehydration, bloody diarrhea and abdominal cramps 2–8 days (3–4 days, on average) after exposure to the organism. While most people recover within a week, some develop a type of kidney failure called hemolytic uremic syndrome (HUS). This condition can occur among persons of any age, but is most common in children under 5-years old and older adults. It is marked by easy bruising, pallor, and decreased urine output. Persons who experience these symptoms should seek emergency medical care immediately.

FSIS and the company are concerned that some product may be frozen and in consumers' freezers.

Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.

FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers of the recall and that steps are taken to make certain that the product is no longer available to consumers. When available, the retail distribution list(s) will be posted on the FSIS website at www.fsis.usda.gov/recalls.

FSIS advises all consumers to safely prepare their raw meat products, including fresh and frozen, and only consume ground beef that has been cooked to a temperature of 160° F. Mary's Home Canning of Lancaster, PA is recalling 516 jars of Mary's Homemade Vegetable Soup (Net Wt. 16 OZ) and 1,128 jars of Mary's Homemade Vegetable Soup (Net Wt. 32 OZ), because it has the potential to be contaminated with Clostridium botulinum, a bacterium which can cause life-threatening illness or death. Consumers are warned not to use the product even if it does not look or smell spoiled.
Botulism, a potentially fatal form of food poisoning, can cause the following symptoms: general weakness, dizziness, double-vision and trouble with speaking or swallowing. Difficulty in breathing, weakness of other muscles, abdominal distension and constipation may also be common symptoms. People experiencing these problems should seek immediate medical attention.

Mary's Homemade Vegetable Soup (Net Wt. 16 OZ) and (Net Wt. 32 OZ) was distributed in PA, MD and DE in retail stores and farmer's markets.

These products were packaged in clear glass jars with white screw top lids in pint-size and quart-size varieties. The label of the jar reads in part: "Mary's Home Made Vegetable Soup...Heat and Serve...829-A Strasburg Road, Paradise, PA 17562." No codes or expiration dates are present on these products.

No illnesses have been reported to date.

The potential for contamination was noted after FDA testing found that the pH of the product was high and it did not receive an adequate process time or temperature.

MIDWEST INTERNATIONAL CORP., 47-57 BRIDGEWATER STREET, BROOKLYN NY 11222 is recalling HOSDO CAKE because it contains undeclared milk. Consumers who have allergies to milk run the risk of serious or life-threatening allergic reactions if they consume this product. In addition, this product was found to contain an unallowed, unidentified green/yellow fluorescent dye.
The recalled HOSDO CAKE comes in a blue and white plastic wrapped, 7 ounce (200gram) package. All manufacturer and expiration date codes are affected. It was sold in New York City. It is a product of South Korea. The UPC code is 6922486392216.

The recall was initiated after it was discovered through routine sampling by New York State Department of Agriculture and Markets Food Inspectors and subsequent analysis of the product by Food Laboratory personnel revealed the presence of milk in packages of HOSDO CAKE which did not declare milk as an ingredient on the label.

No illnesses have been reported to date in connection with this product.

Milky Way International Trading Corp. is initiating a voluntary recall of Nice! Peach Slices and Nice! Mixed Fruit in 8-ounce glass jars out of an abundance of caution due to potential glass in the product. The affected products were distributed to Walgreens stores nationwide and display the lots below. Consumers could potentially be cut or injured if ingested. To date there has been one complaint on the peaches and no complaints on the mixed fruits.

The product has been removed from shelves in Walgreens stores.

The voluntary recall is limited to specific production codes as follows. The lot codes are located at the neck of the glass jars.

The product comes in 10 ounce plastic packages, code information “Best Before 1, July 2016.” Novus Snack Golden Raisins in 10 ounce packages were distributed in Brooklyn NY, and have reached consumers through retail stores.

The recall was initiated after routine sampling by New York State Department of Agriculture and Markets Food Inspector and subsequent analysis by Food Laboratory personnel revealed the presence of sulfites in the 10 ounce package of Novus Brand Golden Raisins which were not declared on the label. The consumption of 10 milligrams of sulfites per serving has been reported to elicit severe reactions in some asthmatics. Anaphylactic shock could occur in certain sulfite sensitive individuals upon ingesting 10 milligrams or more of sulfites. Analysis of Novus Brand Golden Raisins revealed they contained 53.49 milligrams of sulfites per serving.

Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, listeria infection can cause miscarriages and stillbirths among pregnant women.
NORVEN herring in oil was distributed to distributors and retail stores in NY, NJ, CT, IL, PA, MA, MD, VA, FL, and CA.

The recall is the result of sampling and testing by the U.S. Food and Drug Administration (FDA) which revealed the presence of Listeria monocytogenes in the 17.6 oz. (500g) packages of NORVEN Herring in oil, Lot # 530611. The distribution of the product has been suspended while FDA and the company continue to investigate the source of the problem.

WASHINGTON, April 14, 2016 – Perfect Fit Meals, LLC, a Houston, Texas establishment, is recalling approximately 10,455 pounds of poultry entrees due to misbranding and undeclared allergens, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The products are mislabeled and one of the products, the Turkey Enchilada, may contain milk, a known allergen which is not declared on the mislabeled product label.

The Turkey Enchilada items were produced on March 28, 2016. The Lemon Pepper Chicken items were produced on March 25, 2016. The following products are subject to recall: [View Labels(PDF only)]

10.4-oz. sealed tray packages with a label that says “Perfect Fit Meals Texas Chicken Ranch Casserole” with a use-by date of 4/24/2016, but may contain “Perfect Fit Meals Lemon Pepper Chicken.”
11-oz sealed tray packages with a label that says “Perfect Fit Meals Chipotle Chicken Sausage” with a use-by date of 5/2/2016, but may contain “Perfect Fit Meals Turkey Enchilada.”
The products subject to recall bear establishment number “P-827” inside the USDA mark of inspection on the sealed tray packages. These items were shipped to retail locations nationwide.                                 

The problem was discovered after the firm was notified by a retailer that the products were incorrectly labeled.

EBB (U.S.) Inc. of Columbus, Ohio is voluntarily recalling 97 cases of PICs by Price Chopper Cinnamon Rolls with Cream Cheese Icing (UPC 041735073389) because, although milk was listed in the ingredients list, it was mentioned in the wrong allergen caution. This voluntary recall has been issued by EBB (U.S.) Inc. out of an abundance of caution to protect consumers who might be at a risk from exposure to milk.
PICs by Price Chopper Cinnamon Rolls with Cream Cheese Icing were packaged in 12.4 oz. canisters. The product was sold exclusively at Price Chopper stores in New York, Connecticut, Massachusetts, Pennsylvania, Vermont, and New Hampshire. All PICs by Price Chopper Cinnamon Rolls with Cream Cheese Icing shipped from March 7, 2016 to March 28, 2016 are impacted.


To date, there have been no reports of illness in connection with use of this product. The recall was initiated after it was discovered that the product label on the dairy-containing product listed milk as an ingredient but used the wrong allergen cautionary language. Reser's Fine Foods, Inc. of Beaverton, Oregon is recalling nineteen refrigerated salad items due to notification from one of our ingredient suppliers that Listeria monocytogenes may be present in one lot of onions that was used in the manufacture of these salads. Listeria monocytogenes is an organism which can cause serious and sometime fatal infections in young children, frail or elderly people and individuals with weakened immune systems. Healthy people may suffer only short term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea. Listeria infection can cause miscarriages and stillbirths among pregnant women.
The recalled refrigerated salads were distributed to retailers and distribution centers in the following states: AL, AR, AZ, CA, CO, HI, ID, IL, IN, KS, KY, MI, MN, MO, MT, NC, NE, NM, NV, OH, OK, OR, PA, SD, TN, TX, UT, WA, WY, and British Columbia.

The retail product is sold in plastic containers and is marked with a Use By Date and the #10 at the end of the line of printing (see photo below).

No illnesses have been reported to date.

The recall was the result of a notification from one of our onion suppliers that the bacteria may be present in one lot of onions used in the manufacture of these refrigerated salads. No other items were manufactured using this lot of onions.

Consumers are urged not to consume these products. Consumers who purchased these products may take them back to the store for a refund or discard them.

Naples, FL – Back to Nature Foods, LLC is voluntarily recalling two (2) lots of its Classic Crème cookies, 12 oz packages, because it may contain undeclared milk, not listed as an ingredient on the label. Persons who have an allergy or severe sensitivity to milk run the risk of a serious or life-threatening allergic reaction if they consume this product.
The affected product was distributed to retail stores nationwide. Packages are marked with a unit UPC #19898-01103 and with best by dates of 10 SEP 16 and 16 SEP 16 labeled on top of the package. No other best by dates are affected.

Consumers who have purchased the Back to Nature Classic Crème cookies with the UPC and dates noted, and have an allergy to milk, should destroy the product they have or are urged to return to the place of purchase for product replacement or refund.

This voluntary recall is being conducted with the knowledge of the U.S. Food & Drug Administration.

WASHINGTON, April 7, 2016 – Pilgrim’s Pride Corp., a Waco, Texas establishment, is recalling approximately 40,780 pounds of fully cooked chicken nugget products that may be contaminated with extraneous plastic materials, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.

The fully cooked chicken nugget products were produced on Oct. 5, 2015. The following products are subject to recall: [Label (PDF Only)]    

20-lb. cardboard boxes containing two, 10-lb. clear plastic bags of fully cooked chicken nuggets labeled as “GOLD KIST FARMS Fully Cooked Whole Grain Popcorn Style Chicken Patty Fritters.”
The products subject to recall bear establishment number “EST. P-20728” inside the USDA mark of inspection and include package codes 5278105021, 5278105022, 5278105023, 5278105000, and 5278105001. These items were shipped for institutional use to Arizona, California, Colorado, Florida, Georgia, Kansas, Kentucky, North Carolina, Nebraska, Oklahoma, Tennessee, Texas, and Utah.                                

The problem was discovered after the firm received several consumer complaints regarding plastic contamination of the chicken nuggets.  The firm notified FSIS personnel of the issue on April 6, 2016.

There have been no confirmed reports of adverse reactions due to consumption of these products. Anyone concerned about an injury or illness should contact a healthcare provider. 

Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.

FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers of the recall and that steps are taken to make certain that the product is no longer available to consumers. Michel et Augustin Inc. of Brooklyn, NY, is recalling approximately 900 packages of its Petites baguettes butter cookies Dark Chocolate, 6 packs only, because they may contain undeclared hazelnuts. People who have an allergy to hazelnuts run the risk of serious or life-threatening allergic reaction if they consume this product.
Michel et Augustin Petites baguettes butter cookies Dark Chocolate were distributed to retail stores throughout New York City.

The product is packaged in a sealed polypropylene film inside a cardboard box (6 cookies per box), NET WT. 1.58 oz., UPC 812668020166, with a Best before date 07 2016 (located on back of the package).

No illnesses or complaints have been reported to date.
The recall was initiated after the firm discovered packages that had visible pieces of nuts on top of the butter cookies. Subsequent investigation by the firm and foreign manufacturer revealed that a small quantity of packages of the milk chocolate and hazelnut recipe were incorrectly packed into Dark chocolate packages, and hazelnuts are not listed on the label.

Consumers who have purchased Michel et Augustin Petites baguettes butter cookies Dark Chocolate, 6 packs, with Best before date 07 2016 and are allergic to hazelnuts should not consume this product.
